A New Jersey drug maker, Glenmark Pharmaceuticals, has voluntarily recalled thousands of bottles of a combination high blood pressure medication over concerns the product could be cross-contaminated with another drug, the Federal Drug Administration says.
The recall covers more than 11,100 bottles of bisoprolol fumarate and hydrochlorothiazide tablets under the brand name Ziac. Testing of reserve samples showed presence of ezetimibe, a drug used for high cholesterol, FDA officials said in a report published online.
The recall level for the drug is Class III, meaning the use or exposure to the product is “not likely to cause adverse health consequences.”
Glenmark Pharmaceuticals Inc. in Elmwood Park, N.J., is recalling the pills, which are in 2.5 mg and 6.25 mg doses.
The FDA says packages affected are:
30-count bottles NDC 68462-878-30
100-count bottle NDC-68462-878-01
500-count bottles NDC68462-878-05
The affected lot numbers were set to expire beginning in November 2025 through May 2026.

WASHINGTON — Alphabet unit Waymo said Friday it would issue a recall for its self-driving vehicles after Texas officials said they illegally passed school buses at least 19 times since the start of the school year.
The National Highway Traffic Safety Administration, which opened a probe in October into Waymo vehicles near school buses, had asked the self-driving car company on Wednesday to answer questions about the Texas incidents by Jan. 20.
"Holding the highest safety standards means recognizing when our behavior should be better. As a result, we have made the decision to file a voluntary software recall with NHTSA related to appropriately slowing and stopping in these scenarios," Waymo chief safety officer Mauricio Pena said in a statement.
Pena added the company believes software updates that have been implemented meaningfully improved performance.
Waymo said a software issue contributed to self-driving vehicles initially slowing or stopping for a school bus then proceeding.
The recent software update will be applied under the recall, and a Waymo spokesperson said the company "will continue to track and implement more updates as needed."
In a Nov. 20 letter posted by the NHTSA, the Austin Independent School District said five incidents occurred in November after Waymo software updates to resolve the issue.
The school system asked the company to halt operations around schools during pick-up and drop-off times until it could ensure the vehicles would not violate the law.

Bridgestone Americas Tire Operations said in the customer recall notice published on Costco’s website that a lack of a required Department of Transportation certification symbol had prompted certain Bridgestone Blizzak 6 tires in the size "235/40R19" with the identification numbers "1VR100W122224, 1VR100W122324, or 1VR100W122424" to be recalled.
"While the tires fully comply with the performance standards of FMVSS 139, the Department of Transportation has found that tires missing the required DOT symbol may be used in inappropriate applications, increasing the risk of a crash," the company said.

Costco members may have purchased a Danby 8,000 BTU U-Shaped Window Air Conditioner subject to that recall between June 10, 2021, and Feb. 2, 2022, through Costco’s website, per the notice. The model number of the affected air conditioners sold at Costco was "DAC080B6IWDB-6."
"Pooled water in the air conditioners can fail to drain quickly enough, which can lead to mold growth," it said.
To address the issue, Midea is providing impacted customers with a repair kit or a refund that it said will be "based on the purchase data or date of manufacture."
Costco members can also take their recalled Danby window air conditioner units back to the warehouse retailer, where they can get all their money back.
The broader Midea recall affected about 1.7 million units of varying brand names and sizes that were sold in the U.S. at a number of different physical retailers and online, according to the Consumer Product Safety Commission.
Another recall affecting Costco customers involved the Model A1257 "Anker Power Bank (10K, 22.5W)," a recall notice published by the retailer showed.

Ritz crackers recalled in 8 states over life-threatening peanut allergy risk from mislabeling
Peanut butter sandwiches mislabeled as cheese could trigger severe reactions in people with peanut allergies
Ritz Peanut Butter Cracker Sandwiches were recalled in eight U.S. states after a labeling mix-up that could trigger severe allergic reactions in people with peanut allergies.
The Food and Drug Administration (FDA) said the 70 affected cases were shipped to retailers in New York, New Jersey, Pennsylvania, Georgia, Arkansas, Missouri, Oklahoma and Alabama.
The move follows a similar recall in July, though the FDA said this latest action is not an expansion and is being taken out of an abundance of caution. The new recall applies only to the same single SKU and two code dates identified in the summer.
The affected cartons contain individually wrapped packs that may be labeled as cheese even though they are peanut butter. The FDA warned that anyone with a peanut allergy could face a serious or life-threatening reaction if they eat the mislabeled product.
Even so, the outer cartons are labeled correctly and note that the product contains peanuts.
The recall covers 27.6-ounce cartons of 20 Ritz Peanut Butter Cracker Sandwich six-packs. The UPC code is 44000075842, the best-by dates are 8 Jan 26 and 15 Jan 26, and only cartons with the AE plant code on the top of the box are included.
